2024 Player Profile and Parental Consent Form

This form must be completed and submitted if you wish your daughter to take part in any coaching or competitions run by SWG during 2024.

The safety and welfare of juniors in our care is paramount, and it is therefore important that we are aware of any illness, medical condition, and other relevant health details so that their best interests are addressed.

This form must be completed by the parents/carer of any player under the age of 18 taking part in all Surrey Golf activities including Surrey Golf Championships, Knockouts, the Surrey Golf Development Programme, Rookie Tour and JAGS Tour and County Squad players.

In compliance with the data protection act 2018, GDPR and all relevant data protection legislation, all efforts will be made to ensure that information is accurate, kept up to date and secure and that it is used only in connection with the purpose and activities of the organisation. Information will not be kept once a person is no longer involved with Surrey Golf. The information will be disclosed only to those members of the SWG & Surrey Golf for whom it is appropriate and relevant officers of England golf where necessary.

The Player Profile and Parental Consent Form, along with the Code of Conducts are valid for the 2024 season and will be deleted at the end of the 2024 season. Player Profile Forms must be renewed annually.

Player profiles forms enable those responsible for the junior players to have the information they need to deal effectively with any emergency situation that arises.

All information obtained on these forms will be treated as confidential (and only given to those who need it to fulfil a duty of care for the child)

CONSENT OF PARENT/LEGAL CARER/GUARDIAN I give my consent that in an emergency situation, the County may act in my place ( Loco Parentis) if the need arises for the administration of emergency first aid and/or other medical treatment, which, in the opinion of a qualified medical practitioner may be necessary. I also understand that in such occurrence all reasonable steps will be taken to contact me or the alternative adult named on this form.

The submission of this form denotes that my child has my permission to be on the premises where county activities are taking place
I acknowledge that the County is not responsible for providing adult supervision for my child, except for formal junior golfing coaching/matches & competition.
I agree to my child being transported by County representatives to and from venues when she is representing the County, if required and agreed in writing prior to the event taking place
I give permission for Surrey Women's Golf to use relevant date provided by me to contact me about SWG events applicable to my child/children.
By submitting this form, I confirm that I have legal responsibility for the junior (child) named above and that I am entitled to give this consent and am aware how this information may be used.

I confirm that I have read/understand and agree to the terms of the photography policy. I give permission for SWG to publish photographs of my child on their website and/or social media sites in order to promote and celebrate Surrey women and girls
